Horizon Family Dental Care Smile Club
If you do not have insurance, try our Smile Club! This discount dental club can help you and your family receive high-quality dental care while saving money. Individuals and families are both eligible for Smile Club, and children under 18 may be dependents. Members receive yearly x-rays, two dental exams and cleanings per year, and 30% discounts on treatments.
- Member – $250 per year
- Member 1 Dependent – $435 per year (savings of $65 per year)
- Member 2 or 3 Dependents – $600 per year (savings of $150 per year)
- Member 4 or more Dependents – $900 per year (savings of $350 per year)
